Non-thromboembolic pulmonary hypertension in multiple myeloma, after thalidomide treatment: a pilot study

September 25th, 2008 by admin

Background: Multiple myeloma (MM) is thrombogenic as a event of binary astringent personalty and endothelial damage. Thalidomide has been related with an accumulated venture of thromboembolic pulmonic hypertension (PH). PH in the epilepsy of venous thromboembolism has also been described in MM patients during thalidomide treatment.
Aim: Detection of clinical and subclinical nonthromboembolic PH in MM patients after thalidomide treatment.
Patients and methods: Eighty-two patients, 46–82 eld (median geezerhood 61 years), 42 males, were studied. They underwent echocardiographic think at baseline, 1 period thereafter, 6 months after and whenever symptoms indicating diminution of cardiac duty appeared. Echocardiographic signs of PH were especially identified.
Results: Clinical and echocardiographic assessment revealed quaternary patients (out of 82 patients, 4.87%) with PH. Nonimaging and imagery characteristic methods excluded thromboembolic PH. Statistical psychotherapy demonstrated momentous reciprocity between structural hunch disease and PH (r = 14.078; P = 0.008). No momentous reciprocity between geezerhood (r = 0.770; P = 0.724), sexuality (r = 1.157; P = 0.285), International Staging System (ISS) (r = 0.316; P = 0.716) and PH was found.
Conclusions: Preexisted endothelial pathology cod to structural cardiac disease enhances the vasoactive substances promulgation feat accumulated pulmonic tube resistance. Thalidomide mayhap causes a stimulant and vasoconstriction imbalance, which haw drive deviant pulmonic tube salutation meddling to a vicious lot perpetuating PH. (Source: Annals of Oncology)
